Recognition of Serious Incidents and Root Cause Analysis Training

This one day course outlines how to recognise a serious clinical incident based upon National patient Safety Agency and the latest NHS England framework guidance. It guides attendees via case based exercises through the process of performing a Root Cause Analysis investigation of an incident. It also provides guidance upon identifying learning including how Human factors and Systems factors may influence incidents. Guidance is provided upon identifying forms from the solutions and upon writing up SI reports using template forms from the NPSA.
